Thank you for your Access to Health Records Request (A2H).
We have received your request and a member of our Individual Rights Team will be in touch soon for further information if required and to confirm your ID if you do not provide documents outlined below.
Before we can proceed with your request you must provide copies of ID documents as follows via email to [email protected]:
- Your Photographic ID – Photo page of your current passport or Photocard Drivers Licence.
- Your Proof of Address – A recent utility bill (gas, electric, satellite television, landline phone bill) issued within the last three months
- Patient’s Photographic ID – Photo page of your current passport or Photocard Drivers Licence.
In addition to the above ID documents, please provide copies of the following:
If the patient is deceased:
- A grant of Probate or Letter of Administration naming you in the document.
- A copy of the Death Certificate naming you as the Informant.
If the patient is living:
- A signed letter of consent naming you as the information recipient. An acceptable template is available here.
- Power of Attorney – Health and Welfare (Power of Attorney – Finances will also be considered)
If you cannot provide the above ID documents, then other forms of accepted ID are listed on the Gov.uk website. One item from proof of name and one item from Proof of address columns listed here.
Once we receive your ID documents your request will be processed in line with Access to Health Records Act 1990 timescales.
