There are two key types of request for personal information that can be made, a Subject Access Request (SAR) for information of a living individual or Access to Health Records Request (A2H) for a deceased person. 

Subject Access Request (SAR)

If you are requesting information about about yourself please fill out the Subject Access Request form.

After clicking on submit, please email the Individual Rights team ([email protected] ) the following proof of identity to help them process your request:

Photographic ID:

One of the following:

  • Current passport
  • Driving licence
  • Buss pass

And address ID:

  • Utility bill issued within the last 3 months

If requesting information on behalf of a child, please also provide the child’s birth certificate in addition to the above ID about yourself.

Access to Health Records Request (A2H)

If you are requesting information about someone else please fill out the Access to Health Records Request form.

Please use this form if you are requesting information about a deceased patient; after clicking on submit, please email the Individual Rights team ([email protected]) the following proof of identity to help them process your request:

Entitlement to the deceased’s information:

One of the following:

  • Grant of Probate
  • Letter of Administration
  • Death certificate

Requester’s ID

Photographic ID:

One of the following:

  • Current passport
  • Driving licence
  • Buss pass

And address ID:

  • Utility bill issued within the last 3 months
